Prior and/or current insurance information, including:
Insurance company name/employer name
policy/group number
coverage type (Group/Individual/COBRA)
Effective Date/Ending Date of coverage
For any medical questions answered as “YES”:
Attending Physician name, address, telephone number, and fax number, if possible
Specific diagnosis and treatment of condition
Medications taken, including dosage, frequency, and start/end date of use
Duration of condition, including start/end dates
If surgery was performed, a description of surgery/procedures and dates of the procedures
